As a convenience for medical providers, early childhood educators, teachers, and other professionals, Local Speech + Language Services provides referral forms. These can be downloaded, filled out, and faxed to (431) 317-0772 or emailed to admin@localslp.ca. Please include any additional relevant medical records that may assist us in completing a comprehensive assessment and recommended plan.

Please note, you do not have to be referred by a professional. Self-referrals are welcome!
